Complications of Dermatological Treatment

Complications are uncommon if the health professional performing the peel is properly trained.

  • Comedones (blocked pores) or acne may result from the peel itself or from thick moisturisers used afterwards; ask your dermatologist for treatment.
  • Infection due to bacteria (Staphylococcus aureus), yeast (Candida albicans), or virus (Herpes simplex); you may need antibiotics.
  • Scarring may result from infection or picking the scabs, and can be permanent.
  • Blotchy pigmentation is most likely in those with darker skin or who had a pigmentation problem before the peel; keep out of the sun and use hydroquinone.
  • Persistent solar keratoses may require treatment. Your dermatologist may choose cryotherapy, 5-fluorouracil cream or biopsy a lesion in case it is skin cancer.

RESTYLANE:

In a non-surgical procedure, Restylane® or Perlane® is injected into the skin to add volume to smooth away moderate to severe facial wrinkles and folds, such as the lines from your nose to the corners of your mouth (nasolabial folds). Perlane is composed of larger gel particles than Restylane and is intended for deeper injection to volumize skin for correcting facial wrinkles and folds. Achieving the look you desire is as easy as it is quick. The result is instantaneous. No previous skin test is needed, which means that you can have the treatment immediately. The session often takes less than thirty minutes. The time differs somewhat depending on the correction you want to have performed. During treatment, RESTYLANE is injected into the skin in tiny amounts with a very fine needle. When injected into the skin, RESTYLANE produces volume that smoothes the wrinkle or fold, shapes the facial contour or enhances the lips in a natural way. The result is immediate. The method of injecting RESTYLANE is fast and safe and leaves no scars or other traces on the face.

Pain Reduction

For wrinkle treatments, pain relief is not required by all patients; however it can be easily arranged at your request. Prior to your procedure, a topical anaesthetic cream can be applied to the area to be treated. You can also choose to have a local anaesthetic injection.

After Treatment

Immediately after the treatment, you can expect a slight redness, swelling, tenderness and an itching sensation in the treated area. This is a normal result of the injection.The discomfort is temporary and generally disappears in a few days. When it comes to lip augmentation, the swelling may last longer. Some patients experience swelling for about a week. During this time, the lips can look somewhat uneven. This means that the result directly after the treatment should not be regarded as the final result. If the discomfort continues or if other reactions occur, you should always contact your practitioner.

A touch-up treatment within 2-4 weeks after the initial treatment may be necessary to achieve an optimal correction.

As RESTYLANE is a totally non-animal product, there is no risk of eliciting allergic reactions if you are sensitive to common foods, such as beef, chicken and eggs. Unlike some other injectable products for wrinkle treatment and lip enhancement (for example collagen made from cow hide and hyaluronic acid made from rooster combs), RESTYLANE is a totally non-animal product. Scars & Keloids

There can be a number of problems with scars on the skin. A keloid is an excessive production of scar tissue that can result from acne scarring, a surgical scar   or from ear or body piercing. They are more common in darker skin types and certain areas of the body.

Keloids can also be itchy or painful. There are a number of treatment options for a keloid scar which includes injections into the scar, laser or intense pulsed light therapy, topical treatment in the form of creams or occasionally re-excising the scar.

Traumatic scars

These can often be improved by scar revision surgery. This is because a neat surgical scar will heal better than tissue that has been injured, torn or crushed. Scars in some areas can undergo stretch and widen. Once again treatment options are sometimes available for this.

Keloid scars

Occasionally scars enlarge spontaneously to form firm, smooth, hard growths called keloids. Keloids may be uncomfortable or itchy, and may be much larger than the original wound. They can arise soon after the injury, or develop months' later.

It is not known why keloids appear. While most people never form keloids, others develop them after minor injuries, even insect bites or pimples. Keloids may form on any part of the body, although the upper chest and shoulders are especially prone to them. Dark skinned people form keloids more easily than Caucasians.

Keloid scars are usually just a cosmetic problem. They never become malignant.

Hypertrophic scars

As wounds heal, scar tissue forms, which at first is often red and somewhat prominent. Over several months, a scar usually becomes flat and pale. If there is a lot of tension on a healing wound, the healing area is rather thicker than usual. This is known as a hypertrophic scar.

Treatment

Hypertrophic scars generally settle in time but keloids may prove resistant to treatment. The following measures are helpful.

Dressings should be worn for 12 to 24 hours per day, for at least 8 to 12 weeks, and perhaps for much longer.

Self-adhesive polyurethane scar reduction patches

  • Silicone gel dressings
  • Pressure dressings
  • Surgical excision (but may result in a second keloid even larger than the original one)
  • Corticosteroid injection, repeated every few weeks
  • Cryotherapy
  • Superficial X-ray treatment soon after surgery.
  • Pulsed dye laser

Dermatologists at our Dermatology Centre are able to assess and advise on the best management for a problematic scar having all of the above treatment modalities available for use.

Skin Cancer

The term skin cancer usually refers to non-melanoma skin cancer (NMSC) as compared to melanoma cancer that is also a malignant growth originating in the skin.

Types of Skin Cancer

The two most common forms are basal cell carcinoma (BCC) and squamous cell carcinoma (SCC). Both these tumours are related to cumulative sun exposure and most commonly present in an older population with a life-time’s history of sun exposure. With more of us spending an increasing amount of time outdoors and taking holidays abroad we are now seeing these tumours in patients in their 20’s and 30’s. Outdoor workers and people with fair complexions are particularly at risk.

Treatment of Skin Cancer

Non-melanoma skin cancer (NMSC)   presents as either a skin coloured lump, a scaling lesion, an erosion that will not heal or an ulcerated lesion. Any such lesion requires immediate assessment by a medical practitioner. Dermatologists are specifically trained to diagnose such lesions by looking at them and if there is any doubt by performing a biopsy.

A Dermatologist has a number of treatment options available to them which includes the use of special creams to kill the cancer cells, scraping the lesion away to the appropriate depth, cutting the lesion out, freezing the lesion with liquid nitrogen, using photodynamic therapy or occasionally radiotherapy.

The choice of treatment will depend on the tumour type and the personal preference of the patient. 

MOHS MICROGRAPHIC SURGERY PROCEDURE:

Mohs Micrographic Surgery is the most advanced and effective treatment procedure for skin cancer available today. The procedure is performed by specially trained surgeons who have completed at least one additional year of fellowship training (in addition to the physician's three-year dermatology residency) under the tutelage of a Mohs College member.

Initially developed by Dr. Frederic E. Mohs, the Mohs procedure is a state-of-the-art treatment that has been continuously refined over 70 years. With the Mohs technique, physicians are able to see beyond the visible disease, to precisely identify and remove the entire tumor layer by layer while leaving the surrounding healthy tissue intact and unharmed. As the most exact and precise method of tumor removal, it minimizes the chance of re-growth and lessens the potential for scarring or disfigurement.

Because the physician is specially trained in surgery, pathology, and reconstruction, Mohs surgery has the highest success rate of all treatments for skin cancer - up to 99 percent. The Mohs technique is also the treatment of choice for cancers of the face and other sensitive areas as it relies on the accuracy of a microscopic surgical procedure to trace the edges of the cancer and ensure complete removal of all tumors down to the roots during the initial surgery.

Trichology

At the Dermatology Centre we have a Specialist Hair Clinic which is run by a Dermatologist Specialising in all forms of Hair Loss and Hair Disorders.  This means the person you see is a doctor, Consultant Dermatologist (skin specialist) and a hair specialist.  This allows a medical approach to the diagnosis and treatment of all hair disorders and hair loss including investigation of potentially underlying medical causes. Treatment, which may be topical (in the form of lotions) and/or systemic (in the form of tablets) can then be prescribed and obtained from your local pharmacy at market or prescription prices.

How do we differ from a trichologist or trichology centre?

A trichologist does not need to be a doctor and in fact many trichology centres do not have doctors available at their centres.

In addition to his qualifications as a Doctor and a Dermatologist, he has undertaken research with global experts in the field of hair loss and hair disorders and has lectured in the United States of America and Europe.

Warts

Warts are caused by the human papilloma virus (HPV) of which there is several subtypes which affects humans. Common in children of school age, warts can also affect otherwise healthy adults. A wart has the ability to hide from an otherwise normal immune system.

Dermatologists at the Dermatology Centre have a range of treatments to combat warts including newer treatment that encourages the immune system to recognize the virally infected cells and eradicate them.
