Entity Defination Block

Advanced diagnostics : comprehensive laboratory and biomarker assessment ordered beyond the scope of a standard annual physical, including metabolic, hormonal, inflammatory, genetic, and body composition measures, used to characterize an individual's current physiological state and future disease risk while intervention remains possible.

Advanced Diagnostics in NYC: Measuring What a Standard Physical Does Not

A standard annual physical is designed to catch disease that has arrived. It is not designed to characterize the decade of drift that precedes it. A patient can pass an annual physical while carrying insulin resistance that will not show on glucose for another eight years, thyroid antibodies that will not raise TSH for another five, an ApoB that is not reflected in a normal LDL, and a steady loss of muscle mass that nobody measured at all.

Advanced diagnostics measure those things deliberately. Every test we order has to answer a question: what would we do differently if this came back abnormal? If the answer is nothing, we do not order it.

What We Test

1

Metabolic and cardiovascular

Fasting insulin identifies insulin resistance years before glucose moves and is among the most useful and least ordered tests available. HbA1c for glycemic trend. ApoB rather than LDL alone, because ApoB counts the actual number of atherogenic particles and identifies people at risk whose standard lipid panel looks acceptable.

Lipoprotein(a) once in a lifetime, because it is substantially genetic, unaffected by lifestyle, and materially changes cardiovascular risk assessment. Triglyceride-to-HDL ratio as a practical insulin resistance marker. hs- CRP and inflammatory markers.

2

Hormonal

Complete thyroid, adrenal, and sex hormone assessment as described on the hormone optimization program plus IGF-1.

3

Body composition

Muscle mass is among the strongest predictors of healthy aging and functional independence, and it is invisible on a scale. We measure it directly and track it. See why body composition matters more than weight

4

5

Nutritional and functional

Vitamin D, B12, ferritin, homocysteine, magnesium, and omega-3 index. These are cheap, frequently abnormal, and directly correctable — and deficiency here produces symptoms patients often attribute to something far more complicated.

6

Biological age and genetic testing

Epigenetic biological age assessment and genetic panels covering pathways relevant to metabolism, detoxification, methylation, and cardiovascular and cognitive risk. $895. This is longitudinal information — most valuable as a benchmark you track against yourself over time rather than as an absolute verdict. See: biological age vs. chronological age.

7

Multi-cancer early detection

A single blood draw screening for a signal shared across many cancer types, including several with no established routine screening. $1,250. This is an addition to, never a replacement for, guideline-recommended screening — colonoscopy, mammography, and the rest still apply. Full detail on what the test does and does not do: multi-cancer early detection screening

7

Sleep

Where indicated, home sleep apnea testing. Untreated sleep apnea is common, underdiagnosed, and drives fatigue, weight gain, hypertension, hormonal suppression, and cardiovascular risk. See home sleep apnea study.

Why Interpretation Matters More Than Testing

Anyone can order a large panel. The value is in reading it as a system.

A patient with a normal TSH, a low-normal free T3, a high reverse T3, low ferritin, and a flattened cortisol curve does not have five separate findings. She has one story, and the story tells you what to do first. A man with normal total testosterone, high SHBG, low free testosterone, elevated fasting insulin, and central adiposity has a single interconnected problem with an obvious starting point.

Every result in our program is reviewed with you personally by Dr. Gulati, in plain language, with a written plan naming what to address, in what order, and how we will know it worked. We will also tell you which findings do not require action, which is a service in itself — abnormal-looking numbers cause a great deal of unnecessary
anxiety.

The Findings That Change Management Most Often

In practice, a handful of results account for most of the plans we write.

🩸

Elevated fasting insulin with normal glucose.
Extremely common, entirely actionable, and almost never measured outside this kind of program. It reframes a patient's entire risk picture and usually reframes their diet and training too.

📈

Elevated ApoB with an acceptable LDL.
A patient reassured by a normal-looking lipid panel may be carrying substantially more atherogenic particles than that panel implies. This changes cardiovascular management directly.

🫀

Elevated lipoprotein(a).
Largely genetic, unaffected by lifestyle, measured once in a lifetime, and materially significant. Patients with a strong family history of early cardiac events frequently find their explanation here.

🦋

Thyroid antibodies with a normal TSH.
Early autoimmune thyroid disease, years before it would be diagnosed conventionally, and worth monitoring and supporting rather than discovering later.

💪

Low muscle mass at a normal body weight.
Invisible without measurement, and among the most consequential findings for long-term function.

💊

Significant vitamin D or ferritin deficiency.
Unglamorous, common, and frequently the entire explanation for fatigue that had been attributed to something far more complex.

How This Fits With Your Other Doctors

We are not trying to replace your primary care physician, your cardiologist, or your endocrinologist, and we say so to every patient.

What we provide is a scope of measurement and an amount of interpretive time that the conventional visit structure does not accommodate. When we find something requiring specialist management, we refer and we share the data. When you are already under specialist care, we work alongside it and send results to your treating physician.

Patients occasionally worry that findings here will conflict with what another physician has told them.Sometimes the emphasis differs — a value we would act on may be one another physician reasonably watches.Where that happens, we explain the reasoning rather than undermining your other doctors, and we are glad to speak with them directly.

What Advanced Testing Cannot Do

1

More testing is not automatically better.

Every test carries a false positive rate, and false positives generate anxiety, further testing, and occasionally procedures with real risk. We order what will change management.

2

Biological age testing is developing science.

Methodologies differ between vendors, results vary between tests, and the science is evolving. It is useful for tracking your own trajectory. It is not a verdict, and we present it as what it is.

3

Screening is not prevention.

Finding disease early is valuable. Not developing it is better, and that comes from metabolic health, muscle mass, sleep, and the interventions in the other programs — not from the panel itself.

Pricing

Comprehensive biomarker panels are ordered through Labcorp or Quest and billed separately; portions are frequently billable to insurance depending on your coverage and the indication. Biological age and genetic testing is $895. Multi-cancer early detection screening is $1,250. Your initial one-hour evaluation with the physician is $600, and follow-up visit fees depend on visit length and treatment provided.

Advanced diagnostics are included at the Executive Longevity tier of the Longevity Membership Program at $1,250 per month, which is the most economical route for patients who want the full picture annually.

Frequently Asked Questions

An executive physical is typically a one-day battery of tests with a summary report. This is ongoing measurement with interpretation and a plan, tracked over time. The single snapshot is less useful than the trend.

Laboratory panels ordered for a clinical indication are frequently partially covered. Biological age testing, genetic panels, and multi-cancer screening are generally self-pay.

Core panels annually for most people, or every six months while actively addressing something. Genetic testing is once. Lipoprotein(a) is once. Biological age testing is most useful annually as a trend.

You get a plan, and where appropriate a referral, with coordination rather than a printout and a phone number. We work alongside your primary physician and specialists.

No. All testing is available individually. Membership makes it less expensive and puts it in a longitudinal plan.

No, absolutely not. It is an addition to guideline-recommended screening, never a substitute.
