Bioidentical Hormone Replacement Therapy Q & A

Hypertension-Banner

What is hormone replacement therapy?

Our bodies depend on an array of chemicals called hormones to function properly. But as we get older, our bodies begin producing fewer of some hormones that are important for good health. Hormone replacement therapy, or HRT, replenishes the body’s natural supply of hormones to resolve symptoms that occur when hormone levels decline.

What's the difference between bioidentical hormones and other types of hormones used in HRT?

For years, HRT has relied on synthetic hormones to replace estrogen, testosterone and other hormones. These hormones have important structural differences compared to the hormones naturally produced by the body. As a result, they don’t function in quite the same way, and they also aren’t eliminated from the body as easily, causing side effects – some of which can be quite serious. Bioidentical hormones are also created under rigorous laboratory conditions, but they are designed to be structurally the same as our natural hormones. That means the body “recognizes” them, processes them and eliminates the in the same way as natural hormones, so side effects associated with the structural differences in other synthetic hormones are eliminated or significantly decreased.

What symptoms or issues can bioidentical HRT be used to treat?

Bioidentical HRT is used for many issues and symptoms surrounding menopause, including:

It can also be used in patients with other types of hormonal deficiencies or medical issues affecting hormones, like polycystic ovary syndrome (PCOS)

How can I tell if bioidentical hormone therapy is right for me?

The best way to tell if you might benefit from treatment is to schedule an exam and consultation.

What is Bioidentical Hormone Replacement Therapy (BHRT)?

BHRT uses hormones that are chemically identical to those naturally produced by your body. These hormones, often derived from plant sources like soy or yam, are designed to restore hormonal balance and relieve symptoms of hormonal decline or imbalance.

Traditional HRT often uses synthetic hormones, which differ slightly in structure from your body’s hormones. BHRT uses bioidentical hormones that match your body’s own, which may reduce the risk of side effects and offer a more natural approach to hormone therapy.

BHRT can help with:

  • Hot flashes and night sweats
  • Mood swings and irritability
  • Sleep disturbances
  • Brain fog and memory loss
  • Low libido
  • Fatigue
  • Vaginal dryness
  • Weight gain
  • Hair thinning

Anyone experiencing symptoms of hormonal imbalance due to menopause, perimenopause, andropause (male menopause), thyroid issues, or adrenal fatigue may benefit. A complete evaluation and lab testing are required to determine candidacy.

BHRT can be delivered through:

  • Pellets inserted under the skin
  • Creams or gels applied topically
  • Pills taken orally
  • Patches worn on the skin
  • Injections or troches (sublingual lozenges)

Pellets are tiny implants placed under the skin, releasing hormones gradually over 4–6 months. They offer consistent hormone delivery and are convenient because they don’t require daily use.

When properly prescribed and monitored, BHRT can be safe. However, like all hormone therapies, it carries potential risks such as blood clots, stroke, or breast cancer. Regular follow-up and lab testing are essential for safe use.

Most patients begin noticing improvements within 2 to 4 weeks, but optimal results may take 3 to 6 months, depending on hormone levels, delivery method, and overall health.

Blood, saliva, or urine tests are used to measure hormone levels before treatment. Follow-up testing is typically done every few months to adjust dosages and ensure balance.

Yes. Men with low testosterone (andropause) may benefit from BHRT to improve energy, libido, mental clarity, muscle mass, and mood stability.

Some bioidentical hormones are FDA-approved (like estradiol and progesterone), while custom-compounded formulations are not FDA-approved individually, though they are made in regulated compounding pharmacies.

Possible side effects include:

  • Acne or oily skin
  • Bloating
  • Mood swings
  • Breast tenderness
  • Headaches
Adjusting the dosage usually minimizes these issues.

Not necessarily. Treatment duration depends on your symptoms, health goals, and age. Some patients use BHRT short-term, while others benefit from longer use under medical supervision.

Yes. Hormonal imbalance can cause weight gain, particularly around the abdomen. BHRT can support metabolism, muscle mass, and energy levels, which may help with healthy weight maintenance.

Begin with a consultation with a qualified physician. They will review your symptoms, run necessary lab tests, and design a personalized treatment plan with regular follow-ups to track your progress.
