What Longevity Medicine Actually Is

Conventional medicine is built to find and treat disease. It does that well. What it is not built to do is notice the decade-long drift that precedes disease — the fasting insulin creeping up while glucose stays normal, the thyroid antibodies rising while TSH holds in range, the muscle mass quietly declining, the inflammatory markers ticking higher year over year.

Your program begins with a full one-hour evaluation with Dr. Gulati ($600, credited into your first month for members). This covers your complete history, family history, current symptoms, prior treatment, medications and supplements, sleep, training, nutrition, stress, and — importantly — what you actually want from the next twenty years. Physical examination as indicated.

Comprehensive testing

Laboratory work is ordered against your presentation and your tier. Core panels include complete metabolic and lipid assessment with ApoB, fasting insulin and HbA1c, a full thyroid panel with antibodies, complete sex hormone panel, cortisol rhythm, inflammatory markers, and nutrient status including vitamin D, B12, ferritin, and homocysteine. Testing runs through Labcorp or Quest and is billed separately; portions may be billable to your insurance.

Higher tiers add body composition analysis, advanced cardiovascular risk markers, biological age and genetic testing and multi-cancer early detection screening.

Your interpretation visit and plan

This is the visit patients tell us they have never had before. Dr. Gulati walks through what every result means, how the results relate to each other, and where your actual risks and limiting factors are. You leave with a written plan: what to address, in what order, and how we will know it worked.

The plan is specific. It may include medication or hormone adjustment, targeted nutritional intervention, a training prescription, sleep intervention, referral for a sleep studypeptide therapy (PM), hormone optimization, or a structured metabolic and weight program. It will also name the things you should stop doing.

Implementation with support

Between visits you have a program coordinator and a defined path back to Dr. Gulati. Protocols get adjusted when something is not working rather than at the next annual visit. Members receive priority scheduling.

Reassessment and refinement

Repeat testing at defined intervals — typically six months, or three for members addressing something active. Trends matter more than single values, and the second and third rounds of testing are where the program's value compounds. Your plan is revised against what actually changed.
